How to Write a Plan of Correction
A Plan of Correction (POC) is the formal written response a skilled nursing facility must submit to CMS after receiving deficiency citations on the CMS-2567. A strong POC addresses five required elements for every cited F-Tag.
The 5 Required Elements
- Corrective action taken for residents found to be affected
- Identification of other residents with the potential to be affected
- Systemic changes (policies, procedures, in-service education) to prevent recurrence
- Monitoring plan with frequency, responsible party, and QAPI integration
- Completion date for full compliance
