Plan of Correction (POC)
Definition: A written response submitted by a facility to the survey agency describing the corrective actions taken and planned for each deficiency cited on the CMS-2567. Must address five required elements within 10 calendar days of receiving the Statement of Deficiencies.
In Practice
The POC is the most important compliance document a facility produces after a survey. It must describe: (1) what was done for affected residents, (2) how other potentially affected residents were identified, (3) what systemic changes prevent recurrence, (4) how the facility will monitor effectiveness, and (5) the completion date. A well-written POC demonstrates genuine corrective action and can influence enforcement decisions.
