Monitoring Plan
Definition: The fourth required element of a Plan of Correction, describing how the facility will systematically track and verify that corrective actions remain effective over time, including who monitors, what is measured, how often, and what triggers corrective action.
In Practice
An effective monitoring plan specifies measurable outcomes, realistic frequencies (daily→weekly→monthly), compliance thresholds, escalation procedures, and QAPI integration. Vague monitoring statements like 'the DON will monitor' without specifics are a common reason for POC rejection.
