Comprehensive Assessment
Definition: A thorough evaluation of each resident's functional capacity, health status, and preferences using the MDS instrument. Required upon admission, annually, and with significant change in condition.
In Practice
The comprehensive assessment forms the foundation for all care planning. It must accurately reflect the resident's medical, nursing, mental, and psychosocial needs. Inaccurate assessments lead to inadequate care plans and may result in deficiencies related to both assessment accuracy and care delivery.
