Minimum Data Set (MDS)
Definition: A standardized, comprehensive assessment instrument used to evaluate the clinical, functional, and psychosocial needs of every resident in a Medicare/Medicaid certified nursing home. Currently version 3.0.
In Practice
MDS assessments must be completed upon admission, quarterly, annually, and when a resident's condition changes significantly. The data drives care planning, quality measures, reimbursement (RUG/PDPM), and staffing. MDS accuracy is frequently examined during surveys, and inaccurate MDS data can result in deficiencies and payment adjustments.
