Care Plan
Definition: A comprehensive, individualized plan developed by the interdisciplinary team (IDT) that documents each resident's assessed needs, goals, and the specific interventions the facility will implement to meet those needs.
In Practice
Care plans must be based on the comprehensive assessment (MDS) and updated as the resident's condition changes. CMS requires care plans to be person-centered and developed with the resident's participation. Inadequate care planning is one of the most common sources of survey deficiencies.
