Definition: Services provided to assist residents in maintaining or improving their psychosocial functioning and well-being, including adjustment to facility life, discharge planning, advance directives, and addressing behavioral health needs.

In Practice

Facilities with more than 120 beds must employ a qualified social worker. Social services staff play key roles in care planning, resident and family communication, discharge planning, and addressing psychosocial needs. Surveyors assess whether social services are adequate and individualized.
