Definition: A systematic process for identifying the fundamental underlying cause(s) of a problem or deficiency, rather than addressing only the symptoms. Essential for developing effective systemic changes in Plans of Correction.
In Practice
Root cause analysis is a critical component of effective POC writing. Simply re-educating staff without understanding why the failure occurred leads to repeat deficiencies. Common root cause categories include inadequate training, insufficient staffing, unclear policies, lack of monitoring, and communication failures. The QAPI program should use RCA as a standard tool.
