The CMS-2567: Statement of Deficiencies Explained

Form CMS-2567, the Statement of Deficiencies and Plan of Correction, is the official report card of a nursing home inspection. Every deficiency a surveyor cites — and every Plan of Correction the facility promises — lives on this document.

The Two-Column Format

• Left column — the deficiencies: each citation includes the F-Tag and regulation text, the scope-and-severity letter, and the surveyor's evidence from observation, interview, and record review.
• Right column — the Plan of Correction: the facility's response covering corrective action, identification of other residents at risk, systemic changes, monitoring, and completion dates — the five required elements of a Plan of Correction.

How to Read the Citations
