Systemic Change

Definition: The third required element of a Plan of Correction, describing the permanent changes to policies, procedures, systems, or practices that the facility will implement to prevent the deficiency from recurring.

In Practice

Systemic changes must address the root cause of the deficiency, not just the symptoms. Examples include policy revisions, new training programs, equipment upgrades, staffing adjustments, and monitoring systems. The most effective systemic changes demonstrate that the facility understands why the deficiency occurred and has altered the underlying system.
