Definition: The formal document (CMS-2567) issued by the survey agency listing all deficiencies identified during a survey, including the F-Tag number, scope and severity rating, and the specific findings that support each citation.

The Statement of Deficiencies is the official trigger for the POC process. It is a legal document that becomes part of the public record. Facilities receive it after the survey team has completed their analysis and determined final citations. The 10-day POC response deadline starts from the date on this document.
