Definition: An unexpected occurrence involving death or serious physical or psychological injury, or the risk thereof. Sentinel events signal the need for immediate investigation and response.

In nursing homes, sentinel events include unexpected deaths, elopements, assaults, significant medication errors with harm, and other events where the outcome was not related to the resident's condition. Facilities should have systems for identifying and investigating sentinel events and implementing corrective actions.
