Scope and Severity
Definition: A rating system used by CMS to classify each deficiency based on two dimensions: severity (the degree of harm or potential harm to residents) and scope (how many residents are affected). Produces letter ratings from A through L.
In Practice
Scope and severity ratings determine the enforcement actions CMS will take. Levels A-C are typically not cited as deficiencies. Levels D-F indicate no actual harm but potential for more than minimal harm. Levels G-I indicate actual harm. Levels J-L indicate Immediate Jeopardy. The scope dimension ranges from isolated to pattern to widespread.
