Pressure Injury
Definition: Localized damage to the skin and/or underlying tissue, usually over a bony prominence, resulting from sustained pressure or pressure in combination with shear. Formerly called pressure ulcers or bedsores.
In Practice
Pressure injuries are among the most closely scrutinized clinical outcomes in nursing home surveys. CMS uses the Pressure Ulcer CEP to investigate whether facilities are properly assessing risk, implementing prevention strategies, and treating existing pressure injuries. Stage 3 and 4 pressure injuries frequently result in actual harm citations.
