Unintended weight loss, dehydration, and malnutrition are serious concerns in nursing homes and frequently result in survey deficiencies. Facilities must monitor weight weekly for new admissions and monthly thereafter, intervene when significant weight changes occur, and ensure adequate fluid intake. Dietary preferences and cultural considerations must be honored.
