Interdisciplinary Team (IDT)

Definition: A group of healthcare professionals from different disciplines who collaborate to assess, plan, and deliver care for each resident. Typically includes the physician, nurse, social worker, dietitian, activities director, and other relevant staff.

In Practice

CMS requires that care plans be developed by the interdisciplinary team with the resident's participation. The IDT meets regularly to review and update care plans based on each resident's changing needs. Effective IDT collaboration is a hallmark of well-performing facilities.
