Directed Plan of Correction
Definition: A Plan of Correction that is written or dictated by the survey agency rather than the facility, typically imposed when a facility's own POC submissions have been repeatedly rejected or when the deficiencies are particularly serious.
In Practice
A directed POC removes the facility's ability to determine its own corrective actions. The survey agency specifies exactly what the facility must do, by when, and how it will be monitored. This is considered a serious enforcement action and indicates significant loss of confidence in the facility's ability to self-correct.
