Deficiency

Definition: A facility's failure to meet one or more federal participation requirements as determined by a surveyor during an inspection. Each deficiency is documented on the CMS-2567 with an F-Tag number, scope and severity rating, and supporting observations.

In Practice

Deficiencies range from minor (no actual harm, potential for minimal harm) to critical (Immediate Jeopardy). Each deficiency requires a response in the Plan of Correction. The number and severity of deficiencies determine the enforcement actions CMS may take against a facility.
