Definition: A unique six-character identifier assigned by CMS to each Medicare/Medicaid certified healthcare facility. For nursing homes, the CCN consists of the two-digit state code followed by a four-digit facility number.

The CCN is used to identify facilities across all CMS systems, including survey results, quality measures, and the Nursing Home Compare database. It remains with the facility regardless of ownership changes. The last four digits for skilled nursing facilities typically range from 5000-6399.
